Gift Aid Forms

HMRC have produced the following forms which clubs should use to register donations and, where possible, claim Gift Aid on them. If you have any questions or would like further information about the forms then please contact HMRC's Charities Unit.

Gift Aid Documents

Gift Aid DeclarationModel Gift Aid declaration form for individual donors to complete and give to CASC.

CASC Sponsorship DeclarationGift Aid declaration form for multiple donors (i.e. sponsorship).

Official Authorisation FormForm to allow an individual trustee or director to make repayment claims on behalf of the club.

R68 Repayment Form Form used to make a claim for repayment of Gift Aid or tax deducted from bank/building society interest.

R68 Repayment Form Notes Guidance notes to help with the completion of form R68.

R68 Gift Aid (Schedule)Underlying schedule of individual Gift Aid donations (to be submitted with R68)

R68 (F) CASC Underlying schedule of bank/building society interest received and tax deducted (to be submitted with R68)
